8 [1-ounce / 28-g each] semi-sweet chocolate squares, chopped
2/3 cup [160 mL] sweetened condensed milk
1/2 cup [125 mL] chopped nuts [optional]

Melt chocolate with sweetened condensed milk in microwave, on 'MEDIUM', for about 4 minutes.
Stir until chocolate is completely melted and mixture is smooth.
If desired, stir in nuts.
Spread into a wax paper-lined 8 x 4-inch [20 x 10-cm] pan or 9 x 5-inch [23 x 13-cm] loaf pan.
Chill until firm.
